Do not submit protected health information, symptoms, diagnoses, treatment details, medical records, insurance documents, Social Security numbers, or payment details through the public website.
Use of information
Information submitted may be used to respond to inquiries, manage the interest list, communicate with potential members or broker partners, and provide updates about Aurora membership options.
Email-based form
The current public form opens a prepared email message. This is not a secure clinical intake form and should not be used for medical information.
Future updates
Before commercial enrollment or clinical service activation, Aurora will provide final privacy terms, membership terms, telehealth consent materials where applicable, and service partner data controls.
